The opening credits read: "Universal Home Entertainment Productions and Beacon Pictures Present a Strike Entertainment Production".

The back cover: "2003 Universal Studios".

Shouldn't the first three be listed in the Studio field?

And the last be listed as M/C?

Yes to the studios (this was straight-to-video i believe)

I'd be hesitant to add Universal Studios as the Media company.  Seems like they add that copyright to practically everything so i don't think it means anything really.

I note the proof of purchase tags on the back have Universal Studios Home Video which would be viable.

Was there any logo prior to the start of the film?

Or you could leave it empty.

Universal Home Entertainment Productions...this sounds to me like Universal's Direct-to-Video arm, at least back then.
