Shouldn't the first three be listed in the Studio field?
And the last be listed as M/C?

And the last be listed as M/C? Yes to the studios (this was straight-to-video i believe) I'd be hesitant to add Universal Studios as the Media company. Seems like they add that copyright to practically everything so i don't think it means anything really. I note the proof of purchase tags on the back have Universal Studios Home Video which would be viable. Was there any logo prior to the start of the film?
